All official Trilogy Health Services COVID-19 communications will be sent from this platform. This communication will be sent regularly and will house the latest versions of policy, documentation, forms, etc. that directly impact COVID-19.  Please refer to the items below to access the most up to date information, as this information is subject to change.  These documents can also be found on the K Drive under 04 Clinical/COVID 19.

Logs

  • Screening Log
    • Updated 3/17/2020 - reprint and discard any other versions
    • Securely update and store at the screening table
    • Anyone getting screened must complete log prior to moving to the screening questions, including visitors and employees

      • It is mandatory for all employees with an upcoming vacation, trip, or taking any PTO to complete this survey.  A member of the Clinical Support team may be in contact with you to ensure that you take proper precautions to keep your family, coworkers, and residents healthy.  If planning multiple trips, please submit a new form for each trip.
    • Return From Travel Form
      • It is mandatory for all employees who have returned from vacation, trip, or PTO to complete this survey.  if you answered 'No' to all of the survey questions, please return to work as usual.  You will not receive follow-up from out Clinical Team.  If you begin experiencing symptoms, please immediately contact our Compliance Hotline at 1-800-908-8618.

Based on the issues arising from the current COVID-19 virus, and IRS policy, the Trilogy Foundation is able to assist qualifying employees with some emergency food and childcare assistance.  Please click here to see the eligibility requirements for both programs.
