Five Centura Health hospitals receive Medicare 5-Star Rating for quality

In its February hospital quality ratings release, the Centers for Medicare & Medicaid Services (CMS) recognized five Centura Health hospitals with 5-Star ratings for overall hospital quality:

This marks the sixth consecutive 5-Star rating for MRMC, the second for PKR and the first for AAH, SANHC and SASMC since Medicare began releasing the data in July 2016.

These hospitals are five of only 16 in Colorado to receive a 5-Star rating for overall hospital quality in the most recent release. Nationally, only 455 hospitals — just 13.6% of 3,355 rated by CMS — received a 5-Star rating. The most common overall hospital quality rating is three stars.

“Every member of our team plays a role in creating a culture of safety and reliability. These ratings are a testament to the committed physicians and caregivers across our connected ecosystem who strive each and every day to provide the best possible patient experience,” said Shauna Gulley, MD, Chief Clinical Officer. “Safety isn’t just a clinical care goal at Centura. It is a system priority because we know every community, every neighborhood, every life deserves to be whole and healthy.”

The overall hospital quality star ratings are based on a composite score from seven categories containing a total of 57 hospital quality measures. Outcome categories include mortality, safety of care, readmissions, patient experience, effectiveness of care, timeliness of care and effective use of medical imaging.

Forty-nine hospitals in Colorado were rated by CMS in the most recent data release.

For more information about the CMS hospital quality ratings, visit the agency’s website.

Centura Health hospitals recognized for ‘A’chieving excellence

Safety First, People Always is at the forefront of everything we do as we strive to deliver health care that is compassionate, top-quality and safety-focused. One of the many ways we measure our commitment to safety and high-performing whole person care is through the Leapfrog Group Hospital Safety Grades.

The Leapfrog Group is an independent national watchdog organization driven by employers and other purchasers of health care committed to improving health care quality and safety for consumers and purchasers. The Safety Grade assigns an A through F grade to all general hospitals across the country based on their performance in preventing medical errors, injuries, accidents, infections and other harm to patients in their care.

Centura Health is pleased to announce the nine hospitals that received an A rating:

Our incredible caregivers continue to focus on delivering excellent clinical outcomes and creating an environment of continuous learning. As our ministry dealt with the COVID-19 pandemic, our people remained focused on delivering health care in a different way – compassionate, top-quality and safety focused. Thank you for your continued energy, focus and commitment to living Safety First, People Always!

“An ‘A’ safety grade is an elite designation that your community should be proud of,” said Leah Binder, president and CEO of The Leapfrog Group. “The past year has been extraordinarily difficult for hospitals, but Centura Health shows us it is possible to keep a laser focus on patients and their safety, no matter what it takes.”

Developed under the guidance of a national expert panel, the Leapfrog Hospital Safety Grade uses up to 28 measures of publicly available hospital safety data to assign grades to more than 2,600 U.S. acute-care hospitals twice per year. The Hospital Safety Grade’s methodology is peer-reviewed and fully transparent, and the results are free to the public.

Med Rec May continues

Centura Health recognizes that improving medication safety is one of the most challenging aspects of health care. It involves ensuring accurate information about our patient’s home medications, including clinic visits, emergency room visits, admissions and discharges.

As a care provider, you are essential to the medication reconciliation process. To best equip our clinical stakeholders with updated information on workflows and Epic build, Information Technology (IT) is excited to launch annual Med Rec May which will include required refresher training (developed based on feedback from key clinical stakeholders across all areas of the care continuum).

On May 3, you will be assigned refreshed Med Rec training in LEARN. This week we are looking at the updated list of discontinue reasons. As you complete training, take note of this important topic:

Discontinue Reasons are cleaned up and changing. Centura’s pharmacy leadership has worked in collaboration with nursing, and provider (inpatient and outpatient) leadership to refine the options for discontinuing a home medication. The new options will be in PRD on June 1. See the list of the discontinue reasons below:
  • Patient can’t afford medication
  • Patient not on protocol
  • Dose/agent changed
  • Reorder
  • Duplicate
  • No longer indicated
  • Error
  • Formulary change
  • Therapy completed
  • Unable to determine reason
  • Patient reports not effective
  • Anesthesia stop
  • Discontinued by another provider
  • Contact move-error
  • Unwanted reaction
  • Patient transfer
  • Has not taken in over 30 days
  • Reorder
  • Not available
  • Returned to ADS
  • Stop taking at discharge
A Discontinue Reason will be required for all users, except providers. To help provide clarity and context around the actions taken on home medications, a discontinue reason will be required. Based on end-user feedback the Help Buttons have been updated to be those that are commonly used. You can customize these buttons via the Wrench icon.

Over the next few weeks, please watch for additional articles regarding Med Rec May in your local newsletter.

Stroke prevalence and prevention

May is Stroke Awareness Month, an opportunity to raise awareness about the importance of stroke prevention. This week, let’s explore the types of stroke and how to reduce your risk of having a stroke.

What is a stroke or “brain attack”?
Stroke occurs when blood supply to the brain is blocked or decreased. Without blood flow, up to 2 million neurons may die every minute. Every minute lost increases the chance of a person experiencing permanent neurological damage, stroke-related disability, or death. Stroke is a medical emergency that requires rapid treatment.

Types of stroke: Stroke risk factors:
There are some things you can do to prevent strokes. Some risk factors are controllable, and others are not. It is important to understand your personal risk factors for stroke. Talk to your provider about your risk factors for stroke!
My Stroke Prevention Checklist

✔ Find out if you have atrial fibrillation, a type of irregular heartbeat. Follow up with your provider to manage irregular heartbeat.
✔ If you smoke, make a plan to stop.
✔ Control alcohol use and drink in moderation.
✔ Know your cholesterol level. If you have high cholesterol, diet and exercise can help to lower it. You can also speak to your provider about other         methods to control it.
✔ If you are diabetic, follow your doctor’s recommendations carefully to control your diabetes.
