The Centura Health Wellness Incentive Program is designed to create awareness of potential health risks indicated by biometric values and lifestyle choices. The program is currently underway for the 2015/2016 fiscal year. Learn how you can earn wellness incentives up to $1,000 for the next year. This year, there are minimal changes from last year's Wellness Incentive Program: • The Health Screening Results Form is now personalized for easier identification during the results submission process. If you are completing a biometric screening with your health care provider, you will need to print off your own individualized Health Screening Results Form. • All programs — biometric screenings, reasonable alternatives, healthy activity rewards and online health assessments — now have a deadline of March 31, 2016. • Biometric screenings will no longer be offered at Centura Health Laboratories. You have two options to complete your biometric screenings this year: with your health care provider or at an onsite biometric screening event. • The final biometric screening at Mercy will be held on November 19. This is much earlier in the earning period cycle. This decision was intentional and will allow you more time to complete Reasonable Alternative Programs, if needed. Please log in to your Café Well account and register for an appointment time.
You can earn wellness incentives to reduce your Medical Plan premiums up to $1,000 if you: • Complete a biometric screening between April 1, 2015 and March 31, 2016, • Meet targeted biometric measurements OR complete a Reasonable Alternative Program or request an exemption, and • Complete an online health assessment.

• Gym is closed to associates Monday, Wednesday, and Friday from 8:00 - 9:00 a.m. and Monday through Friday 10:30 a.m. - noon. • Excluding rehab class times, the gym is available to associates 24/7. Associates may badge in after hours. • Check availability of the fitness room when the gym is closed.
Please be mindful of the following when using the gym or fitness room: • No muddy shoes • Turn off and put away ALL equipment when done • Clean equipment with wipes when done • Refrain from using the gym if you are sick or ill __________________________________________________________________
It's the last week of the season to order fresh, local produce, meats, and cheese through the Mercy Wellness Program! This new benefit to Mercy associates allows you to order produce online to be delivered and picked up at the Wellness Center. To register, follow these simple instructions: Southwest Farm Fresh Registration · Access the Southwest Farm Fresh homepage, (www.southwestfarmfresh.com). · Click the link at the very top of the page that reads, Learn more here. · Click the Register tab and select Register as buyer. · Under I am, select a consumer interested in purchasing. · Under Customer pickup location, select Thursday: DIRECT- Mercy Medical Center. · Fill in blanks, under organization, please list Mercy. *These steps are important, as they need to know where you work so that they can deliver your produce to the right location (Suite 144, Wellness Center). After you are registered with Southwest Farm Fresh, you can just select “log in” from the main page and click the “product list” to see what is available. PLEASE NOTE: As of now, you are unable to pay with credit card. Associates are asked to pay via check made out to “Southwest Farm Fresh.” When you go to log out, please select “I will pay later.” You will receive an invoice with your order and the delivery driver, Brian, will pick up checks from the Wellness Center upon delivery. Southwest Farm Fresh Cooperative (SFFC) is a locally owned and operated organization, established in 2014. SFFC represents producers in Durango, Mancos, Bayfield and Dolores who also participate in the ownership of the co-op. SFFC consist largely of produce and meats, in addition to James Ranch Artisan Cheese. The program will continue through the fall until produce is no longer grown. The Southwest Farm Fresh Direct Sales program is funded by the “Enrich Colorado Ag Grant Program” from the Colorado Department of Agriculture (CDA). The Enrich Colorado Ag Grant Program was created by the CDA to provide funding for research, technical and operational issues, sales and marketing needs of local farmers and food producers in Colorado. Southwest Farm Fresh Cooperative strives to be a self-sustained program that continues to offer high quality, local products to a growing population and to make local food available to the everyday consumer while supporting local farmers. __________________________________________________________________
