File Submissions to SOM Academic Affairs
Since off-site work began in March, files have been submitted to SOM Academic Affairs via email. This is a significant change from our previous process, which was almost exclusively on paper.
The guidelines below apply to files submitted to your SOM Academic Affairs analysts via email, and will help ensure that we have everything that we need in order to process your files efficiently.
- Please make sure that your file submission is complete and in one continuous PDF file, not in a PDF Portfolio format. If there is a checklist involved, please make sure the contents of the file are in order.
- When revising a large file with multiple pages, please check with your dean’s analyst on whether they would like you to re-send the entire document, or if they are comfortable adding your revised pages back into the file.
- If your file is too large to send through email, OR if you have several distinct PDF files to send via email, you might consider sending your file/files in a .zip format, or upload it to Webfiles and send your analyst a link to the documents. Files with multiple pages must be in one continuous PDF when uploaded to either the .zip format or to Webfiles.
COVID-19 Leave: AP-76 Forms
All leaves of absence must be recorded on an AP-76 Form. Use APP 7-17 and APP 7-18 for approval authority for leaves other than COVID-19 leaves. Please make sure that the AP-76 form is filled out completely.
The dean’s office must receive an AP-76 form for all COVID-19 related leaves; the dean’s signature is required for all COVID-19 leaves, no matter the length of the leave. EPSL and EFML also require the vice provost’s signature.
For COVID-19 leaves, the type of leave is ‘COVID-19 Related Leave’, and the form must note the type of COVID-19 leave taken, and the qualifying reason for the COVID-19 leave. Use the ‘Additional Comments’ area to record this information.
